Master's Application Procedures
Documents 1 and 2 should be uploaded to the Supporting Documents page of the University Application.
Do not upload Document 3. This document needs to be sent to your recommenders. Please do not list recommenders in the recommendation section of the online application.
- In Document 1, upload---Statement of Intent.
- Click here to open the Statement of Intent (Word document)
- In Document 2, upload---Curriculum Vitae/Resume.
- For Document 3, print Letter of Recommendation Form which should be sent to your recommender. The recommender should attach this form to his/her official recommendation letter. Do not list recommenders in the online application. Two letters of recommendation must be sent to the Department. The letters can be sent by applicant or recommenders.

PhD Application Procedures
Documents should be uploaded in the Supporting Documents page of the University Application.
- In Document 1, upload---Purpose Statement
*Be sure to state clear research
objectives and provide information on faculty
you would be interested in working with.
- In Document 2, upload — Current Resume/Curriculum Vitae
